Account Suspended

Looks like I tripped a fraud alert, I was creating a form for new drivers to submit their information.

Upon checking form https://www.jotform.us/form/93355915372160, I found that there were two violations to our terms.

1). Form is requesting for password which is not allowed

2). Form is asking for sensitive information like Drivers license or SSN. For forms asking this type of information, you need to comply with two things.

1. Add Terms of use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities.

2. Add a Signature widget where your users sign and agree to provide such sensitive information.

I went ahead and removed the password field and reactivate your account. Please make sure to add the two widgets suggested above to avoid your form being flagged.
