Form not found - Form has been disabled

I am getting a Form Not Found - This form is disabled error, even though it is showing enabled. Please advise!

Your form is disabled because your account is suspended. The account was suspended because of the Disputed charge on Stripe. At this moment we can not reactivate your account until this issue is resolved. You need to contact your bank, withdraw the dispute and provide us a proof after the disputed is withdrawn. Only then we can reactivate the account.
