has stopped working. I've not changed anything in the page code - can you let em know what I need to do in order to get it working again?
I have checked your site and was not able to replicate on the said problem. I have checked it also with several other browsers and was still not able to replicate with the said problem.
Can you please try to clear your browsers cache and see if you are able to view your form.
I can't submit a response on your form page - the button says "please wait" and hangs there (I tried in Safari and firefox)
My reply is:
I get the error instantly on any browser I try, and any computer. My client does too. Mac /PC / safari / explorer / firefox.
You see the name/email fields and submit button, but as soon as you hit 'submit' you get the error page.

I was now able to replicate on the said problem. Can you please try to re-embed your form? I have tried embedding your form on a black page and was able to submit without any error, so re-embedding your form should fix the issue. Also, I would suggest using iFrame to prevent any script conflict errors that may have caused this issue.
