I have created a form here: http://form.jotform.us/form/31026554755151
On the first page of that form, there is a field called "Total Number on Team". Depending on the number chosen on that field, there are 30 form collapse sections on the next page of the form. These sections are
If the the "Total Number on Team" is set to 10, then there should be 10 of the form collapse sections on the next page due to the conditions I have setup. I have named these sections "Team Member 1, Team Member 2, etc). They have a spot for name, DOB, SSN.
The problem is that somehow the conditions aren't working. There are 30 conditions - 1 for each form collapse section. When I go to the next page, it is blank with none of the form collapse sections showing.
I did a test and changed 1 of the conditions in the list of 30 conditions to be based on a different field in my form. When I chose the value that corresponded to that condition, all of the correct form collapse sections showed up. It seems to be something with the way I did the conditions and having 30 conditions connected to the same field?
Can you please advise?

Thanks for contacting us. To fixed that, you should use the Conditional Logic settings to like this;
Use "Greater Than" then show each field one by one. Repeat each process for the rest of the Team Member Field;
...and so on...
I've cloned your form and made this changes until Team Member 5. Please kindly check: http://form.jotformpro.com/form/31074592129959.
