    I've copied and pasted the source code to my website, but it only shows up as html. How do I get it to show as a readable form?

    Hello Linda,

    I saw your previous post about the same issue and I can also see that you made the changes I suggested there. However the form is still not rendering correctly on your website.

    May I ask which web authoring software are you using? Please ensure that you are pasting the embed code using the "HTML mode" of your web authoring tool. Also, make sure the embed code is pasted within the <BODY> </BODY> tags.

    It is also a possibility that javascript is not supported by your web authoring tool. To remedy that, you can use the code below (which does not contain any scripting codes) and let us know if it works this time or not.
