Form Disabled Notice

    Hello -- I'm building a new form for scoring Girl Scout Gold Award proposals for my council.  However, I'm getting notices that my form is disabled.  Apparently I have somehow violated terms of agreement, as the settings for my form show that my form is enabled.  Please help. I've reviewed the terms of service and have not identified how I am out of compliance, with the possible exception of using terms copyrighted by GSUSA such as Gold Award and Go Gold or using language from GSUSA's Go Gold platform.

    If it IS the copyright issue, all I can say is that our desire to use these is legitimate.  For further explanation of why I'm developing this form:  Girl Scouts wanting to earn the organization's highest award must submit a Gold Award project proposal for approval.  In each council, there are carefully selected individuals who "score" the proposals as part of the approval process.  In my council, the volunteer committee involved is using a scoring rubric to score proposals.  We were hoping to put the rubric  on line via JotForm as that would make things SO MUCH easier for the committee responsible for approving projects and Gold Award applications.   So we established an email (GSKGoGold@gmail.com) for easy communication among committee volunteers and now are trying to develop a JotForm to use for scoring on line.  There is nothing about this form that will be publicized beyond the volunteer committee. 

    Thank you so much for your assistance; I would at least like to understand what's happening here.

    Your account was suspended. I have reactivated your account as of now. However, I am not able to find what caused this issue. Although I suggest you to insert "Terms and Conditions widget" detailing why you are creating this form.
