I was able to replicate the issue on my end. I have inspected the form and was able to see that you have integrated it with Mailchimp.
However, there seems to be an issue with the integration due to a missing/unmapped field as shown below:
Please try editing the integration and re-map the field intended for the CONSULTATION DATE.
