How do I create multiple column forms?

I figured it out. You use the shrink utility.

You hit the spot. The Shrink function will enable you to make multi-column forms.
For radio buttons and checkboxes, there is an additional option called "Spread Columns" which allows you to arrange the options according to the number of columns of your choice.
Simply click the Spread Columns button in the toolbar's Properties tab, then enter the number of columns you want in the form. Then click OK.
