    How can I combine 2 entry boxes on a single line, and repeat it ?

    I need a form to capture airline ticket numbers linked to a specific passenger name eg:

    Pasenger Ticket number

    SMITH 589-894525478562

    JONES 589-874512458785

    JOHNSON 589-874589526521

    LINCOLN 589-985632145474

    WELLINGTON 589-784125474110


    So the form shuld have a line to collect both name and ticket number for a single passenger, and I don't know how to do that - I'd rather not have this on separate lines.

    You could actually use the Full Name field for this and change its sub-labels from First Name and Last Name to Last Name and Ticket Number respectively:
