    How to add additional feilds?

    I'm creating my own Job application form. I require two fields which I'm not sure how to make with this form creator: Social Security Field and a Desired Salary field. I'm a bit picture perfect, so I'm trying to get it to look as best I can. I'd like to know if there's any way I can create my answer feilds to look something similar to this:


    Social Security Number:    [field] - [field] - [field]
    Social Security Number:    [000] - [00] - [0000]

    Or

    Price:   $[field].[field]
    Price:   $[25].[00]

    The [field] represents the input text boxes.
    Can someone please help? :) It would be much appreciated!

    I think you should just have a reguar text box for SSN and Salary questions. Having them in separate fields is also not very useful for you as well.

    Technically what you describe is not possible. At least on the form builder. You can add them as separate questions, get full form source code for the form and edit the layout in your favorite HTML editor.
