Not receiving report e-mails (howeve test e-mails work fine)

    I have tested your form and was able to receive the form notification. To test a notification with a different email address , you will need to do an actual submission instead of using a test email button.

    I would also suggest checking your spam inbox. Below is the test I made which I was able to received the notification.

    BTW, your hover messages are impossible to read they flick on and off really quickly
