I need the customers name / email to show as a header on the responses instead of jotform
please help

Hi,
To my understanding, you want to put the customer name and email in the Page Browser Title of the form.
Do you have any form samples we can check and try to replicate?

You can add the name and email field in the email subject so when you receive the email notification, it is the name and email of the submitter that is shown in the email subject.
Here's how:
1. In the form builder, go to Settings > Emails > edit notification
2. In the Email Subject, click on the pencil icon to add fields in the subject line. Make sure to save the changes after that.
