Let's say I have a smal doctor office and want to have some web forms that collect my patients information and store it in a database. Do you create the form for me or you give me the tool that I create it myself? If you create it for me, then there is only one time charge or I pay monthly. If I pay monthly, what serives you cover?

You will create the form using our drag and drop form builder which is pretty easy to use. Moreover, Jotform support is present when you need help during the creation. Just post the query on the forum (like what we are doing now) and we will get back to you as soon as possible.
Form creation is free. You can create unlimited forms.
Free Accounts: only allowed 100 submissions a month
Paid Plans: Vary from 1000 to 100,000 depending on the plan.
