Cant get into my account

for some reason when clicking on the link to my form it comes up "404 page not found" and when i go to login to my account after putting in my username and password i click "login" and nothing happens 

Upon review of your account, I found that it was suspended because the forms in your account are requesting Social Security Number and/or Driver's License Number on plain field texts. However, I notice that the necessary Terms and Signature widgets are already added to the form and the account email address is registered with your organization email that is required as per our terms of usage in order to request SSN/DL.

Please note that it is not allowed to collect any credit card information using plain text fields. Due to our PCI certification, we are required to deny the collection of credit card numbers on regular forms.

However, we have other PCI compliant ways to do this, all you need to do is to choose any of the payment processors available in our platform in order to integrate in your forms.

We have now activated your account and you should be able to login to your account without any issue. Please refrain from adding such fields to your forms to avoid any further suspensions. You may now login to your account and manage your forms.
