Error on JotForm form builder

Hello, please see the error I am receiving. See the attached image. I am receiving the error when testing it. Not sure what do to from here?

Unfortunately, I am not sure what integration you are seeing this issue with, it is not displayed in the screenshot that you provided. I have taken a look at your profile and found only one form and I see that it is very different from the form in the screenshot. I also have noticed that you have 2 integrations already up and running.

If you could kindly provide us with the URL of the form in question or simply the integration that you are seeing this issue with, we can investigate further. We look forward to your response!
