Download Form Document Issue

Hello - I'm trying to download a document from a form submission and I continue to get the attached error message. Please assist. 

To troubleshoot the below form was created and it will provide a clear example as to the problem

Unfortunately, I am unable to replicate the issue you are currently experiencing. When I check your submissions I am able to see and download the attached files successfully.

Please note that there are a number of users currently experiencing issues with their submissions page as it is a server problem on our end. Our development team has been notified and is working on a fix this very moment.

In the meantime, I would suggest that you clear your browser's cookies and cache, then log into your account once again and try to access your submissions page. If the issue persists please do let us know so that we may add this thread to our count of users experiencing issues. We look forward to your response.
