I have been using JotForm for almost a year on a personal account, rafolkers, and for several months for my work, nciomr. I have no problems accessing and filling out forms; however, for the past few weeks, first on the nciomr account and then on the other, the site has not displayed properly, and I am not able on either account to access completed forms. The site acts irratically and does not display properly. I am enclosing a screen shot.
These problems have been consistent on several Macs, have been the same after I cleared the cache, and have been the same whether I am using Safari, Firefox, or Chrome

We apologize for this inconvinience. But, currently we don't have any other reported issue similar to this.
I have cleared your jotform account's cache on our end. I hope this resolve the issue for you.
Can you please try again and update us of the result.
Per chance, have you also tried it also on other hardware/computer? Perhaps there are also issue on the current hardware/OS you currently used.
