Do I need to create a mobile version form for my mobile version website?

Hi If i have a mobile version of my website can i copy the same for into both pages or do i need to create 2 separate forms?

You can use the same form. Just embed it on your mobile version website and your form should automatically adjust as it's already mobile responsive.
