Hello I attempted to sign in to my account and it stated that it was suspended. Please reinstate.

Your account was suspended because you collect credit card details in some of your forms. Please note that this violates our TOS. You can read it here:
If you need to collect payments, you should add some of our payment integrations. You can check them all here:
https://www.jotform.com/integrations/category/payment-processing
If you want to reactivate your account, we would need to delete the offending fields.
Please tell us how to proceed.

the vendor (i.e. Carnival Cruise, and other travel vendors) through me. I'm
requesting that they fill out an authorization form to protect them and
myself.

I have deleted the offending forms and reactivated your account.
Please note that you can use "credit card authorization" in the title of the form. But you can not create plain fields to obtain this information.
However, in order to achieve this, you can use some of our payment integrations. Payment authorization is available on the following payment gateways:
- Stripe
- PayPal
- Square
- Authorize.net
