Display one field per row in mobile view

I would like place the fields one under one in mobile view (desktop version is ok). How I can do this?

I tried to simulate your form using Galaxy S5 and it is showing the fields one per row.

The form by default is mobile responsive unless you have added custom CSS that overrides is responsiveness.

Thank you for answer. But in preview mobile mode it's look like this. Why is there difference?

When I viewed the form in my mobile phone, the fields are stacked at each other.
