We apologize for the inconvenience that may have caused. I've check your form and I found out that the issue is had something to do with your form's email notification which is not automatically updated.
I just want to let you know, that once you made some changes with your form like adding or removing fields, you need to manually update your email notification or create new email notification.
So, in order to fixed this issue, please remove/delete your current email notification and create a new one.
