    I just tried making a test submission to your form and was able to download the PDF after submitting the form:

    May we know what browser are you using to fill out the form so we could replicate the issue on our end?

    Also, I have inspected the form and saw that some of the fields on your web form is not connected with the ORIGINAL PDF. I suggest manually adding a SHORT TEXT ENTRY for the names and a SIGNATURE field for the signatures:

    After that, connect the said fields to the fields of the original PDF. Here's an example:
