Sorry for the inconvenience that may have caused. Your form and account is working fine upon checking and I wasn't able to replicate the issue. Please take a look visual bellow.
What internet browser you are using now viewing your form? Have you tried already using other browser?
I currently use Chrome checking your account and form.
