Blocked IP Address & Forms

    This is the second time my stuff has been blocked. I followed what was previously asked of me and was told everything would be up and running. What needs done so I can get my forms back online? This is for my job and have customers needing to submit their information.

    I can see you were previously assisted regarding the same matter on the following thread: https://www.jotform.com/answers/2124402 

    On that thread you were informed that your account was suspended due to asking for SSN and drivers license numbers on your forms. 

    Upon checking your account and it seems to have been suspended due to the following form where you are asking for the same information: https://www.jotform.com/form/200206066803141

    I can see you have added the terms and conditions widget as per my colleague suggestion, but the signature element is still missing, you may refer to the thread linked above to get the guide about adding a signature element to your form. 

    I have reactivated your account now so you can edit your form as needed, if you have questions, let us know.

    I was able to update the forms but now my IP address is blocked again.

    I've just checked your account and IP address, none seems blocked, but I've whitelisted the current IP address. If you're still blocked, please get your current IP and share it on this thread, this link will help you getting it: https://whatismyipaddress.com/
