    It seems your account Fustos was suspended due to the form 200321874423346 which was flagged as possible phishing form.

    We understand this might not be the case with your form, yet we had to act according to the general rule to be in compliance with our Terms: Non-corporate emails showing suspicious forms must be suspended.
    Also, our reviewers checked your account IP address and it is form Hungary but the website INTERDREAM is from Netherlands.

    In line with this, we would like to request these information from you:
    1. The URL where your forms are used.
    2. Your website or proof of affiliation to a legitimate institution.
    3. Share with us a document that will prove that you're authorized to use the said logos.

    I use the logo mine

    no web page

    JotFor trying is good for me what do I need?

    has worked so far

    did not hurt anyone

    I sent company papers

    I paid for JotForm 

    you make others page?
