    My account is suspended. How do I get access?

    You've been suspended due to the credit card fields in your form. It's against our terms of use (https://www.jotform.com/terms/). If you need to collect payments at a later date, you need to integrate forms with our payment gateway like Paypal and enable Payment Authorization.

    The other fields such as the ID number and Passport numbers are allowed, but you need to have the terms and signature widgets in it. 

    I've reactivated the account, but I have deleted the CC fields.
