Scheduling form: Need a form where previous entries are not selectable

Hello,

I'm looking for a form with a memory. Like appointment slot choosing.

For instance if I have 10 openings this week, if that user (or another user) chose a date yesterday, I want that date to be unavialable or greyed-out today.

It could also be used as a potluck sign up sheet or gift registry where there was a fixed list but each chosen item removes that item from the form for future users to chose from.
