Somehow my form doesn't seem to be under my account. I don't think I have a second account... Could it have disappeared?Page URL:

Apologies for the inconvenience caused, I can see four forms in your account but can not determine if the form you created is part of it. Please provide us the form's name/url.
If you are not able to find the four forms please clear your browser's cache then log back in into the account.
