Nothing happens when I hit "submit". Form goes blank.

I was actually checking the Thank you page message to see if it is causing the issue. However, as I check the form in the edit mode, the Submit Application button is set by the Page Break instead of a Submit button. 

This is why the form is displaying the second page instead of submitting the form. Please try removing the page break and add Submit button to the form so that it should be working fine.
