    Hi Andrew,

I checked your from and see that the required fileds under "Delegate Handouts" are missing. The three radio button options seems not to be there.

Here is your form's direct link: http://www.jotform.com/form/92163336267

Let us know if you need further assistance.

    Thanks,

    Sidharth

    Hi sid,
I guess I must have removed the option boxes on client request. strange
that this has not been an issue until now.
Can I just edit the form at my account in JotForm to remove the option
boxes - and will it then work without me messing with my html?
    Kind Regards, Andrew

--- OR is there a way to disable the field in JotForm so I could bring it
    back into use if wanted?

    @bizshows

You can just change this field not to be required and that should solve your issue.

I just checked and see that you have removed the options from your form. I did a test submission successfully.

It seems that your issue is resolved.

    Thanks,

    Sidharth

Sid - thanks so much. A speedy resolution. Again, though, not sure why
    this started happening suddenly.

    You are welcome.

It seems that the fields must have been made required.

Anyways, glad to know that your issue was resolved.
