Need your assistance right away

On both our access form for Vehicle Audit and PRU Audit the form is supposed to copy the unique code from the date and the vehicle number however it applies the same number to every request which causes us issues.  Can you  please have a look as it has been fine up until today and I cannot fix it.  It should change based on the vehicle that is selected.  For example https://form.jotform.com/90585960758271?session=18314.21 the last two number should be different for every vehicle selected.  2041 should read 18314.2041 and for 2101 is should read 18314.2101 and so on.  Can you help to fix this please. 

I have tested your form and was able to replicate the issue, I cloned the form and ended up resolving the issue by changing the condition, kindly set up the condition as follows:

I have to just say...I have been a member of this jotform program for over 5 years in 2 different accounts and I am impressed with the level of assistance you and your team provide for us.  Great work.  I also changed the date field to A-Z and it now looks like it should with the date and the unit number forming this unique link.  Thank you so much for your quick response.
