I can't log in and the form I created is "diasbled"

    Hello, 

    I created a free account and saved a form yesterday - can't even log in today. Changed the password with resent password, but it says SUSPENDED when I try to log in. Please help.

    Hi Sam,

    I have checked your account and it seems auto suspended by our Phishing detector because your form contains a field that is asking for SSN. You can collect this type of information however you need to add the mandatory widgets:

    1. Terms and Conditions or Short Scrollable Terms widget.

    2. E-Signature widget 

    Also, you need to change your email address to a corporate one as proof of legitimacy and that you are affiliated to an institution that provides the services you are offering on your form. Please be guided by our Terms of Use to prevent further issues.

    To reactivate your account, we need to delete the offending forms. Let us know how would you like to proceed regarding this matter.
