    Hello, I can't login to my to my account on Jotform (username: prazenica),

    because login failed - SUSPENDED.

    Please allow me access to my account.

    Your account has been suspended due to your form being tagged as scam, offering fake insurance service which is against our Terms Of Use.

    If this is not correct, kindly provide the following details:

    1. Company name

    2. URL where the forms are used

    3. Corporate email - you will have to use this in your Jotform account.

    Alternatively, we can simply delete your form.

    This form has been used as a preparation for future contracts, the form is now obsolete. Please remove the form, thank you.

    As per your advise, I have now purged the said form and reactivated your account prazenica.
