Firstly, may I say that the JotForm service is second to none for designing custom forms, and is a breeze to use. I have designed and adminster dozens of Joomla based websites, and have decided to add a quick contact form for each site using the "Enquiry Form" code. My idea was to have a simple button on every page, and make contact as easy as possible. The first website, I ceated a custom HTML module assigned to to a module position, and pasted the code while in HTML mode. Great result, a quick contact button on the left of each page. I then tried the second website, and despite having created the same custom module, no button appeared. I then inserted the code into Jumi, still not joy. I then insrted the code into individual web pages (having no editor selected) and still no joy.I was convinced that the problem lay with different configurations withing Joomla, although all the sites began life as a standard installation.
All the back end work was done using Chrome. Having walked away puzzled for an hour, I then returned, and tested the site in Firefox, and hey presto, three buttons, one being the custom module, one being the Jumi module and the other being the code embedded into the pages.
Everything else appears to work, whether just the base code embedded into a page, or a lightbox popup with a link. The problems seem only to relate to the Enquiry Form code.
Has anyone experienced similar problems?

Based from what I've read in your post, it seems that the cause of your problem is an external Joomla plugin (or is it a script?) that is not related at all with Jotform. Is that correct?
As much as I would like to help you with that, I have to admit that I know little to nothing about that particular script and as such there's not much I can offer.
I believe that the perfect place where you can find solutions from people who share the same interest with Joomla is the Joomla Forums.
