Jot form is asking for permission access to a form I emailed to employees.

Why is it requiring sign-in access?

If the form was assigned to specific user/s, on the users in the Assigned List will be able to access the form.

If you do not want the form assigned, you can remove the list of assignee in Publish / Assign Form option.
