I can't login to my account even after resetting the password.

    Unfortunately, you cannot use a textbox to collect the credit card information/bank details as it is against our terms. Your Forms/account might get suspended due to this.

    I am unable to login to remove those fields: please would it be possible for you to remove the fields and reinstate my login?
