How can I unsuspend account

Hi, 

How can I unsuspend my account please so that I can continue using Jotform.  

The form is no longer on my website and I wish to add in the terms and condition and signature widgets you have suggested last week

We have suspended your account, because you collect bank information and require a file upload of ID. Please note that this is not allowed and it violates our TOS. You can read it here:

If you need to collect payments, you should use some of our payment integrations. You can find them here:

If you do not collect this data from the general public, but from your own clients, you can collect IDs and driver's licenses. You need to comply with two things:

1. You should add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your clients to read and agree.

2. You should add a Signature Widget. So, your clients can sign and agree to provide such sensitive information.

Please have in mind that it is only allowed to ask for driver's license and ID numbers. You can not ask for any kind of file uploads for these documents.

If you want to reactivate your account, you would need to delete the offending fields.

Please tell us how to proceed.

Can you reactivate my account so I can delete the bank details page.
Also I believe I have put on the terms widget and the signature widget as requested but unless my account is reactivated I cannot change anything.

I have deleted the offending fields and reactivated your account. So, you can now login again and use your form.

You should have in mind that you can not ask for any kind of file uploads for personal documents. Please refrain to do this.
