    Hello!

I had previously created a form titled" General Patient Form". It was based on an uploaded PDF with fields that would be populated by JotForm. However, now it no longer populates the generated pdf. Instead it shows an empty version of the PDF when viewing a Submission.

Is there a setting that I need to update or revert in order to have this function again?

    Thank you.

I checked your account and I am assuming that you're referring to this form: https://www.jotform.us/form/93083950299165

Upon checking the form, it is a fillable form. To view the submissions, please follow this guide: https://www.jotform.com/help/269-How-to-View-Form-Submissions

To edit the PDF document in the PDF editor, please follow this guide: https://www.jotform.com/help/384-How-to-Customize-PDF-Submissions-Report

Let us know if that's not what you're looking for. Thank you.

I was able to replicate the issue when I downloaded the Original PDF. I will report this issue to our developers. We will inform you via this thread once this issue is resolved.

For now, I can only suggest downloading the data using the new PDF Document you created in PDF Editor.
