Is this free ? For how long

Hi I just wanted to know firstly if I can make a self employed contract with my contractors and if someone can help me make it 

and secondly using this jot form service is free for a certain time period and then monthly or annual subscription??

You can use a free account until you reach any of your account limits, once you reach a limit, you're free to upgrade or wait for the limit to reset the first day of the next month in EST time.
