I can't use or view my form because it says Form Not Found Form is in Restore Mode

I haven't been on the site or built a form in awhile and it seems my account is dormant, although I was able to build a form I can see it or embed it into my website.  Please help.  Please reactive my account to good standing so that I can view/use the form.

The Form in Question is the: Horizon Christian School Authorize.net Payment Form
