Autoresponder is not going out to the person who submits the form.

I really don't know what I am doing wrong?  I need my auto responses to go the people that complete my form.  Currently it is going to one email address. Please help.    I have set up conditions.

Thank you for contacting us. The way the conditions have been set up must be blocking the Autoresponders. Please note that if you want every person who fills the form to be notified via email, you don't need to set up conditions.

You may delete them, and the Autoresponder will be sent to the email address entered in the form email field called "Want to know your results?".
