I have tried to use the captcha in your JotForms many times. However it never worked. I read all your FAQs, Forums and etc.
I'm and advanced computer user.
I have made my website through Dreamweaver.
I tried JotForm for free in this first step. Thank God I did it in this way, because it really doesn't work as I need.
My OS is Windows Seven Ultimate 64bits and I also have the all updates and both IE9 as well as Google Chrome.
From the all sollutions and suggestions you gave in your FAQs and Forums work, no one helped me.
If no solutions from your side, to solve it immediately, I must say good bye to you for ever.

I cloned your form and using Chrome as my browser, I was actually able to submit the form just fine. The captcha is working ok. Can you please provide us with the URL of your actual site so that we can further review the issue?
The captcha on the direct link on your form, http://www.jotform.com/31457311867659, also looks ok.
One thing I noticed about your 2 conditions on the Notification Email is that they're exactly the same. But please do provide us with the URL so we can check. Thanks!
