Integration with a WP page with conditional settings

I'd like to integrate a jotform I build using into our existing WP site.

I downloaded the three jot form plugins however....... The form is not in a blog but rather a page. When I posted it the whole format, not the beginning questions that then have a format occurs. Obviously looks far less than optimal. The page can be found at:   I want the conditional settings to be part of the experience, not showing the whole form...

I have checked the link that you provided and I am not finding any JotForm there. However, since the website you are working with is a WordPress site, I would suggest you use the following user guide to assist you with embedding your form with WordPress: How-to-Embed-a-Form-to-Wordpress-com

Following the steps in the above user guide should allow you to embed your form on the website with full functionality.
