Reached the maximum submissions - Now our form has disappeared?

    We upgraded about a week ago from the free jotform to the premium ($9.95/month) in order to have access to more form submissions per month. We did this because we recieved an e-mail saying that we had reached our maximum number of submissions. 

    Unfortunately, since we up-graded (May 23rd) our form has dissapeared from our website. We have tried to load the form back up onto the website but had no success. 

    Is there any way to get the form back up on the website?? If so, how could be go about doing this?? 

    Any Help would be great!!

    Get the form's code from the Embed Form Wizard

    and embed it again, using the method you previously used to embed the form when the account was at the Free (Starter) level.

    (The wizard can be loaded from the Setup & Embed tab. Click the Embed Form button, seen above the tab in the screenshot below.)
