my account was disabled

my account was disabled a while ago due to it being suspected as a phishing account, id like to have it reactivated please as its been a year.

Your account is suspended due to the form "90212334669860 GRANT APPLICATION FORM".

Could you let us know the purpose of this form? Could you also provide us any proof to show that you are authorized to collect/offer grants?
