The recording widget works fine to record but won't playback for the person filling out the form and it won't play back for me when reviewing

I tested it and I was able to record my voice. I used an Android phone, may we know what mobile device are you using so we can test it on our end?

Also, if the form is embedded on a website, please share the link so we can check it further.

I am using an iPhone. It seemed to record but I couldn’t play it back. When I received the completed form on my computer it didn’t play. It is not embedded in a website.

I have tested the form and could not replicate any issue with the widget.

The recordings are getting played. I have tested with my iPhone 6s phone. Are you getting the confirmation that the voice was recorded successfully while you are recording the voice?

It seems you have removed the widget from your form. Can you re-add it in your form so that we could test your form?
