    Form Alignment Problems

    I am having a problem with the forms not aligning correctly and html code appearing within the forms. I am using the html form code because the other embed options are not showing up in mobile devices and simulators. I have attached an image showing the problems in several browsers.     It's a conflict between the one of the page's stylesheets (this one) and the form's styles that's causing this issue. I'll see if I can track it down and get back to you.

    In your web page, please replace the form's CSS with this and let us know if the result is okay.
