My account got suspended

Hi Team,

I have login into Jotform by using my gmail. After started creating form i cam to know that my account was suspended. Later i created another jotform with another mail id but result is same even after given the details like "The purpose of creating form or  my company name etc...,". So please let me know my mistake. Thankyou

The form "SIGNUP" is asking for login information which violates our terms of use. That's the reason why your account got suspended. As per our terms, we do not allow collecting login information here in JotForm. Also, the word "Password" will get your account automatically suspended because of our anti-phishing detector tool.

I have now deleted the offending fields from your form and activated your account. Please refrain from creating such forms or adding any guilty fields to the forms to avoid any further suspensions on your account.
