1. It could be due to a temporary server issue. You can download the PDF here for now https://www.jotform.com/pdf-editor/91955596855276?submission=4621293803212997883.
Also, I suggest you add a PDF Link into your email so in case there's no attachment, you could still download the PDF within your email by clicking the PDF link. Here's how:
Go to SETTINGS > EMAILS > Edit NOTIFICATION
Click Form Fields and add the PDF Link.
2. I checked in the server logs and this email is sent. Please check it again on your end including your spam folder. There is also an autoresponder email sent to the user.
