It appears that your form was created under a Guest account. Would it be possible for you to browse to http://www.jotform.com using the same computer and browser that you used when you created that particular form?
That way, you'll be able to access that form again. Also, you may want to register a Jotform account using that same computer/browser. This will enable you to have complete control and access to your form wherever you may be.
